Community Mental Health Services

Community mental health services treat people living in the community aged between 18 and 65 who are experiencing mental health problems.

The Access Team will be the point of access for all mental health referrals, undertake assessments, liaise closely with GPs and provide short term treatments.

The Recovery Team will care for those with longer term more complex needs who require ongoing support.

Your support package  within the access or recovery team may include a personal budget to give you more choice and control over how some of your needs are met. For more information about personal budgets visit the Live it Well website.

Teams are made up of social workers; community psychiatric nurses; occupational therapists; technical instructors; Support Time and Recovery Workers; consultant psychiatrists; clinical psychologists and medical staff and administration and secretarial staff.

Crisis Resolution Home Treatment Team

The Crisis Resolution Home Treatement Team (CRHT) offer a service 24 hours a day 365 days a year. They provide services in response to crisis with community based assessment and treatment. They act as a Gatekeeper to inpatient care and offer an alternative to hosptial admission and facilitate early discharge.

Acute Inpatient Wards and PICU Facilities

Acute admission wards provide inpatient care with intensive support for patients in periods of acute psychiatric illness who can not be supported at home by the Crisis Resolution Home Treatment Teams or Community Mental Health Team. Patients who are in an acutely disturbed phase of a serious mental health disorder, are detained in designed Psychiatric Intensive Care Unit (PICU) facilities.

Admission to acute inpatient wards is through our Crisis Teams.

What our patients say

The Patients Council Monitoring Form (questionnaire) is designed to identify areas of good practice and issues for concern at the Little Brook Hospital, and is independently carried out.  The questions are updated regularly and the results are collected, analysed and quarterly reports are published.  They are distributed to patients and ex-patients of the Unit, nursing and other staff, management of Kent and Medway NHS and Social Care Partnership Trust, West Kent Primary Care Trust, Dartford, Gravesham and Swanley MIND, the Advocacy Network and other interested parties.

Patients Council Workers are employed by Invicta Advocacy Network to assist patients at the Unit to fill in the forms.

Rehabilitation Service

Rehabilitation Services aim to prepare patients to become independent and live fulfilling lives in the local community. Inpatient units are staffed on a 24 hour basis. These provide care to people with complex mental health issues.
