We operate 24 hours a day, seven days a week.
Eligibility criteria
We work with adults of working age (18 to 65 years) and those adults over 65 years who have a functional illness (not dementia).
The team is designed to gate keep all admissions to hospital and provide an alternative to hospital admission where appropriate, by offering home treatment . The team also facilitates 'early' discharge from hospital through the provision of home treatment.
Referral procedure
All admissions are managed through the local CRHT Team and they can be contacted 24/7 on 01622 725000.
Appointments
Appointments for this service are generally not applicable, but any appointments are care plan reviews arranged by the team members.
