1. Introduction

1.1

When a Serious Incident occurs, there must be systematic measures in place to respond to them. These measures must protect patients, staff and visitors and ensure that robust investigations are carried out, which result in organisations learning from serious incidents and putting actions in place to minimise the risk of the incident happening again.

1.2

Adherence with the policy supports KMPT in its objective to pursue continuous improvement in the delivery of its services,  whilst being person-centred, acting openly, fairly and proportionately within just culture.
