Management of investigations and serious incidents policy
The purpose of this policy is to ensure that risks associated with serious incidents are identified and managed in accordance with best practice and in line with the expectations of the NHS Resolution, the Health and Safety Executive, the Care Quality Commission, NHS England and NHS Improvement, Clinical Commissioning Groups and the public.

Introduction
When a Serious Incident or potential Serious Incident occurs, there must be systematic measures in place to respond to them. These measures must protect patients, staff and visitors and ensure that robust investigations are carried out. This in turn must result in the organisation learning from Serious Incidents and putting actions in place to minimise the risk of the incident happening again.
