Pathfinders

The Pathfinders is a network for people living with a diagnosis of dementia in the Swale area.

We are registered with the Dementia Engagement Empowerment Project (DEEP) Network and supported by KMPT staff.

The group aims to make Swale more dementia friendly, reducing stigma and raising awareness on how to live well with dementia.

Examples of what we do:

  • Taking part in a 10k Memory Walk for the Alzheimer’s Society.
  • Supporting the post-diagnostic support group Living Well With Dementia that is coordinated by Swale Community Mental Health Services for Older People.
  • Raising awareness of the personal experiences of dementia in students by recording podcasts for The University of Stirling’s new Masters Postgraduate Diploma.
  • Inviting our local MP to join a meeting to discuss dementia awareness.
  • Supporting the recruitment of staff at Kent and Medway NHS and Social Care Partnership Trust.

We meet once a month in Sittingbourne, to discuss topics related to our aims and goals.

We welcome new members to our group and remain open to any new ideas and contributions.
