If you’ve recently received care from us you may receive an invitation asking you to take part in the national patient mental health community survey.

The survey is sent only to a random selection of those that have used our services. If you’ve received a copy of the survey to complete please fill it in.

It’s your opportunity to feedback about your recent experience(s) of using our services, what they were like and how they could be improved for patients in the future.

The results are vital in helping us as a local healthcare provider to get detailed patient feedback on the standards of our services and care we provide. The feedback is used to help set key priorities for delivering a better service for our patients. The closing date for the survey is Friday 21 June 2019.
